A discharge planner is the hospital staff member, usually a nurse or social worker, who arranges what happens when your parent leaves the hospital.
When your parent is admitted, someone on the hospital team is responsible for planning the exit: a discharge planner or case manager. They line up where your parent goes next, what services follow, and what equipment is needed at home.
This person is your best ally for the handoff. Ask them early what the plan is, whether your parent qualifies for rehab or home health, what medications changed, and what warning signs to watch for at home.
Discharge is a risky moment. Instructions get missed, medications get confused, and follow-up appointments fall through. Pushing the discharge planner for a clear, written plan is one of the most useful things a family can do.
