Polypharmacy.

Polypharmacy is being on many medications at once, which raises the risk of side effects, harmful interactions, and mistakes.

Polypharmacy usually describes taking several medications regularly, often five or more, though the number matters less than the effect. The more drugs on the list, the more chances for one to interact badly with another or to cause side effects that get mistaken for a new problem.

It builds up quietly. Each prescription made sense on its own, added by a different specialist over time, but nobody stepped back to look at the whole list. Confusion, falls, dizziness, and fatigue can all trace back to the combination.

The fix is a periodic review with a doctor or pharmacist who looks at everything together and asks whether each drug is still needed. Families can help by keeping one complete, current list and asking for that review.
